How to access this plan:

Call a Customer Care Consultant and provide your group name and I.D. as indicated on your membership card. Identify your location, zip code and type of doctor required. Provide a telephone number, fax or e-mail should Customer Care need to call you back with provider options.
For physician visits planned in advance, a Customer Care Consultant will contact your chosen doctor prior to your visit.
You will then be free to make an appointment to visit the healthcare provider and pay for services provided at the time of service or per payment terms agreed upon prior to your visit.
